Service Description

A comprehensive assessment of your complete medication regimen by a board-certified clinical pharmacist.  Each and every one of your medications will be evaluated for its safety and effectiveness as well as affordability. Before the appointment, we would like for you to have all the prescription and non-prescription drugs ready, including vitamins, dietary supplements, and other over-the-counter drugs. You will be able to ask and discuss any problems, questions, or concerns you have about your medications with the pharmacist. After the appointment, the clinical pharmacist will email you a complete medication list and medication-related action plan that you can share with your healthcare team.
